Polymyxin B and trimethoprim ophthalmic combination is used to treat bacterial infections of the eye including conjunctivitis (pinkeye; infection of the membrane that covers the outside of the eyeball and the inside of the eyelid) or blepharoconjunctivitis (infection of the membrane that covers the outside of the eyeball and the inside and outer parts of the eyelid).

Polymyxin B (polymyxin b sulfate) sulfate is a drug of choice in the treatment of infections of the urinary tract, meninges, and bloodstream caused by susceptible strains of Ps. aeruginosa. It may also be used topically and subconjunctivally in the treatment of infections of the eye caused by susceptible strains of Ps. aeruginosa.

The combination of polymyxin B and trimethoprim comes as a solution (liquid) to instill in the eye. It is usually instilled once in the affected eye(s) every three hours (maximum of 6 doses in 24 hours) for 7 to 10 days.

Systemic Infections
IV: 15,000-25,000 units/kg/day divided q12hr; not to exceed 25,000 units/kg/day
IM: 25,000-30,000 units/kg/day divided q4-6hr
Intrathecal: 50,000 qDay for 3-4 days; then qDay or qODay for at least 2 weeks after CSF cultures are negative and normal for glucose content
Total daily dose not to exceed 2,000,000 units/day
Renal Impairment
CrCl >20 mL/minute: give 75-100% usual dose/day divided q12hr
CrCl 5-20 mL/minute: give 50% usual dose/day divided q12hr
CrCl <5 mL/minute: give 15% usual dose/day q12hr
polymyxin b side effects
- Dizziness.

polymyxin b mechanism of action
Polymyxins cause cell damage leading to cell death by disrupting the membrane integrity. This mechanism is also at play when the polymyxins interact with the membranes of the human cells, disintegrating them leading to toxicity.
